Most passengers never touch the controls
Executive rear seats offer adjustment that the majority of passengers never use, usually because they do not want to appear fussy or do not know the controls exist. The other reason to adjust early is that discomfort is cumulative. A position that feels fine for ten minutes can be intolerable at ninety, by which point changing it is a relief rather than a comfort. Set it properly at the start. The order that works
Adjust in a sequence rather than fiddling randomly. Start with the seat base: far enough forward that your feet rest flat, with the cushion supporting the whole thigh rather than ending halfway along it. Then the backrest, reclined slightly beyond upright so the hip angle opens. Then lumbar support, increased until it fills the hollow of your lower back without pushing. Finally the headrest, positioned so the back of your head meets it without your chin dropping.
That order works because each adjustment changes what the next one needs. Setting the headrest first and the backrest afterwards means doing the headrest twice. Working through the sequence in that order takes roughly two minutes and then holds good for the rest of the journey.

Changing position during a long journey
No single position is comfortable indefinitely. The most effective habit on a long run is to change something every forty minutes or so: a small change in backrest angle, a shift in lumbar support, or raising a footrest slightly. Movement is what keeps a back happy, not the perfect position.
Massage and ventilation functions where fitted are worth using for the same reason. They shift pressure points and keep circulation moving, which is more useful than the gimmick they are sometimes taken for. Circulation, not cushioning, is what decides how a back feels after three hours.
Passengers with specific needs
Anyone with back trouble, a recent injury, a hip replacement or a leg in a cast has particular requirements, and they are much easier to meet if described in advance. Sometimes the answer is a specific vehicle with more upright seating; sometimes it is a cushion; sometimes it is simply making sure the seat can be got into without twisting.
The front passenger seat is a resource that is usually ignored. Moving it fully forward opens up the space behind considerably, and it is available on almost every booking because nobody is sitting there. Asking for it costs nothing and produces more space than most seat adjustments can.
